Pain and nausea in the stomach, and pain and rushing of blood to the head, with drowsiness, which lasted several hours. Next morning I rose at 5 o’clock, and soon after I think in less than an hour, was suddenly attacked with severe pain and nausea, together with a convulsive bearing in the stomach, resembling the deathly sickness of pregnancy, but without vomiting; this was also attended by faintness and flashing to the head, which made assistance necessary to remove me to a chamber. I think the symptoms lasted a full hour, and were succeeded by partial loss of voice, soreness of throat, and occasional throbbing of the heart. Nausea and faintness, accompanied with a sick, griping pain in the stomach, and loathing of food. The next day at noon, just as she returned from school, she was seized instantaneously with the most acute pain in the stomach; her mother saw her looking perfectly well, and in less than five minutes, found her sitting drawn together with her knees against her breast, and a face like a corpse; deathly pallor; lips blue. She said, “Oh, mother, I feel so strangely, I think I have the cramp”. Cham. relieved, but there remained great vertigo and nausea; she complained of being very cold, but did not shudder; great weakness, as after long illness, with headache lasting all afternoon. About 3 1/2 and 4 o’clock, P.M., sitting in church, she was seized with a sharp, steady pain in the stomach, accompanied by the most deathlike faintness she ever recollected to have felt, with extreme sickness and immediate loss of strength. A cold sweat covered her; she thinks she must have been deathly pale, for she felt as though the blood (which had at first been driven to the head by the suddenness and severity of the pain), were leaving it and the extremities altogether; they were quite cold; her sight grew dim, and she became so faint that she expected every moment to fall from her seat. The pain in the stomach felt as though it would draw her double. Having no friend near her she feared to make the attempt to leave the pew, lest she should not have strength to reach the door unaided, much less her home. The attack lasted, she thinks, about half an hour, after which the violence of the pain gradually abated, the faintness passed off, the chilliness gave place to heat, and the blood rushed to her head with such a face, that she could hardly hold it up, or keep her eyes open, though quite conscious of the services going on about her. After services she was able to walk slowly home, though very weak feeling very much as if she had been ill for a month. She felt the weakness especially in her back although she had no pain there, but during the rest of the evening, whenever drawing a long breath it appeared to come from a spot on each side of the spine, about the middle of the back, come from a spot on each side of the spine, about the middle of the back, on a line with the inferior angles of the scapulae (fifth day). The effects passed off with a night’s rest, and she had no further return of the symptoms (nor has she had such an attack since, two years after). At 3 P.M., while sitting, a sudden jerking pain from the stomach up into the swallow, at the same time a twitching of the lower lip, and weak feeling in the pit of the stomach; he was to rise and walk about when all the symptoms vanished. At 8 P.M., when walking in the open air, a severe pain to the left of the scrobiculum, somewhat relieved by standing still and pressing on it, and disappearing when bending double; when continuing his walk on it, and disappearing when bending double; when continuing his walk the pain return, but a little lower than in the first place and becomes more perceptible with every step. Pressing pain in the pit of the stomach, followed by throbbing sensation, when lying on the back in the stomach, followed by throbbing sensation, when lying on the back in the evening (first day) (second day). In the forenoon, at 11 o’clock, a sinking sensation at the stomach; after eating something she felt stronger, but had for fifteen minutes a violent throbbing, fifteen minutes after which it got better (twenty-second day). Relaxed feeling in the abdomen (third day). Weakness. Accumulation of wind in the abdomen. Distension of the bowels. Taken internally in large doses, Alum excites nausea, vomiting, griping, purging, and even an inflammatory condition of the intestinal canal; effects which may perhaps be induced by small quantities in persons endowed with unusual or morbid sensibility of the stomach and bowels (Pereira). In small doses it always causes a rarer, dryer, and harder stool; in somewhat stronger doses irritation, more exudation and copious stools (Vogt). The bowels confined. In the afternoon, difficult passage of a hard stool, with some blood (the first day). In frequent attacks, first hard, then soft stools, and after it, for three hours, the most violent pains, burning, shooting, and particularly cutting in the rectum upwards, lasting from morning till noon; with the stool passes a great deal of blood, without any relief; sometimes a sensation as if the anus would protrude. After the patient had carried a piece of Alum in his breeches pocket, he never had it again. (Reported to Dr. Neidhard). The same symptoms have in many cases been cured by giving high potencies. Severe pains in the anus and rectum, appearing several minutes after stool; stinging and tearing up the rectum, relieved by bending forward and sitting bent forward; particularly worse when lying on the side; not so much when lying on the back; burning and throbbing in the anus; after five days better, but then a stitching pain from the upper part of the left chest through to the back, between the shoulder blades; worse at night in bed; it wakes him; increases when lying on the left side; not increased by taking a long breath; at the same time, sudden attacks of flutterings at the heart, disappearing as quickly as they came. Ulceration of the rectum; if there is an ulcerated surface, and mostly exceedingly painful excrescences and fetid, bloody ichor, 2M (Wolf). Hemorrhoidal tumor after a hard stool. Piles, with aching within the anus (after eight days). A small lump at the anus itches violently, particularly in the night, not so much during the day. Itching at the anus all evening (first and second day). Urine Increased secretion of urine. Frequent urination. The urine becomes remarkably acid from the use of it (Kraus). In the morning a bluish, iridizing film on the urine passed the evening before. Brown, muddy, urine, over which is formed a thin, variegated cuticle, like that which is formed over stagnant water that contains iron (after six days). The urine looks as though cheesy milk had been stirred into it, b. White, cloudy urine (after ten hours). Orifice of urethra itching. Male Genitals While at dinner (2 o’clock), erections; something quite unusual (third day). In the night an emission of very thick semen; felt unusually well and strong next morning. A cutting in the left side of the penis. Shooting pain from the middle line of the belly, as along a thread down into the penis (third day). Itching on the scrotum and at the orifice of the urethra, in the evening (second day). Female Genitals In the highest degrees of weakness of uterus, its good influence is only transient (Hufeland). Higher potencies relieved permanently if indicated by the symptoms. Causes induration and scirrhus uteri, if continually used for copious menstruation and hemorrhages (Hufeland). Catamenia very scanty and more watery than usual (33 years old), has usually very profuse menstruation. During catamenia hands weak, drops things easily. Menses did not appear since several months, no good digestion, loss of strength, pain in the lower limbs, particularly at night; the column uteri swollen and puffed up, the vagina very sensitive, and narrowed, particularly at the orifice, by swelling of various size (Jacquot). As she had it during pregnancy, faint weakness in pit of stomach. Respiratory Hoarseness, voice shrill, cannot talk loud; at times she feels as though she had lost her voice; altogether the sound of the voice varied when talking, it is sometimes high, sometimes low; cannot talk loud enough. After a week, repeated the proving with precisely similar though less violent effects, except that her voice was entirely lost for a full week, and the soreness of the throat became so troublesome, as apparently to give her doctor some uneasiness; he call ed repeatedly and administered medicine several times during the ensuing fortnight. This aphony lasted for months. Hoarseness; worse from talking, which likewise causes a tickling cough. Hoarseness with tickling cough; with cough in the evening. Talking increases hoarseness; voice changing; throat dry; causes a tickling cough. Larynx Tickling in the larynx causing cough (Trbg). Tickling in throat causes cough; scratching like dust in larynx. Painful scraping sensation of roughness in the larynx, with expectoration of much clear mucus, for two or three days after taking the medicine. With the cough, scraping in the throat to the middle of the sternum; rawness in the breast; but slight expectoration. In larynx, heat and burning. Sense of constriction in the windpipe. Breathing Oppression of chest; tightness across the upper part; sense of heat and burning. Sensation as if ligated in chest. With every deep inhalation, pain on both sides of the spine, in the middle. When inhaling, pain in clavicle. Rapid short breathing with uneasiness in stomach. Respiration rapid. Ineffectual urging to take a deep breath, by fullness in pit of stomach. Not increased by deep breathing, stitches from chest to back. Cough Cough from tickling in larynx, caused by talking. Some irritation to cough, in the morning after rising (first day); tickling in the throat causing cough (second day). Tickling cough one hour after getting up in the morning (second day). Cough immediately after rising, excited by a tickling in the throat; worse during breakfast, after breakfast better, and only occasional cough during the day. Tickling cough in the evening, from twilight (7 o’clock), until bedtime, with hoarseness. Severe spell of hard shaking cough, caused by a scratching sensation in the larynx, as if dust were in it, which relieved by the discharge of a quantity of clear mucus (after a few hours). Dry cough in the evening after lying down; after she felt nearly well; but she had a cough in the evening. Every morning (as often before), some cough (twenty-second day). Cough in the morning immediately after rising, excited by a tickling in the throat, particularly worse during breakfast, afterwards better; only now and then during the day. The same tickling cough in the evening, from 7 o’clock until he goes to bed, with hoarseness. When coughing, a scratching in the throat, to the middle of the sternum, and rawness in the chest; very scanty expectoration. After the 30 cent., immediate improvement; after three days a relapse; then with a repetition of the dose for two days, gradual improvement. Diarrhoea with frequent, violent urging; slimy stools, with severe pains in the rectum during stool, going down into the thighs. A young man who had been given up several years before as an incurable consumptive, but had been afterwards cured by homoeopathy. With the cough, scraping in throat. When coughing, a scratching in the throat, to the middle of sternum. Cough, with pain in region of ovary. Expectoration Slight expectoration; very scanty. Thirst, phlegm in throat and on palate. Expectoration of a mass of clear, transparent phlegm. Expectoration relieves scraping. It evidently so changed the expectoration as immediately to cause a copious discharge of mucus from the bronchi, and finally checked the superabundant secretion; did not produce thirst or any unpleasant symptoms, and so far from aggravating the febrile state, it often served materially to alleviate it. Chest When inhaling, pain under the right clavicle and right shoulder (second day). Since yesterday, stinging from the upper part of the left chest, through to the back, between the shoulder-blades. Worse at night in bed, when it awakens him; not aggravated by deep inhalation. Stitching pain down the middle of the chest; later in the right side. From upper left of chest stitches through to the back. Pain from heart to lower part of right lung. Sore pain in the right side of the chest under the nipple, especially when bending up double (contracting the body) in the morning (second day). Pain in the left chest when sitting, especially when bending forwards. Pains in the chest to the left, near the middle of the sternum, after rising, when washing and dressing, lasting until 9 A.M. The following morning the same and worse (Trbg). Pains in the chest; on side of chest. Rawness on the chest. To the middle of sternum, scraping, scratching. 1 and 10. Sensation of fullness in the chest; in the afternoon, particularly after supper, painful, as if distended in the chest and arms. Soon after taking it in the evening, felt a slight pain or binding sensation in her breast, especially on the right side, retiring to rest almost immediately, it disappeared after lying down. Tightness across upper part of chest. Feeling of weakness in the lower part of the chest and in the abdomen. Heart Half an hour after the second dose he feels very warm in the left side, opposite the shoulder, inside, and feels also very tried, still he slept afterwards very good. To the heart a throbbing from epigastrium. From the heart a pain shooting to the lower part of right lung. At times quite well as if nothing ailed her, then, when beginning to think about her disease, she feels the heart beating, so that it seems to move the whole little stronger than natural, and at other times its motion is scarcely perceptible; sometimes the contractions are frequent, sometimes seldom. The contraction seems to end with a snap, as if the valves of the heart were jerked suddenly shut. Fluttering of heart, sudden attacks, disappearing as quickly as they came. Rapid and violent palpitation, worse when lying on the right side; also when lying in one position for any length of time; has to throw himself about in bed (twenty-first day). Perceptible strong palpitation and strong pulsation through the whole body, when standing for some time on one spot. Was in about an hour seized with a throbbing of the heart, similar to heavy blows of a hammer, with long intervals; in ten or fifteen minutes the violence abated, and gradually subsided. Violent palpitation; intermitting beating of the heart. The palpitation of the heart and other symptoms, except the constipation, were better during the first week until Saturday, when owing to a sudden excitement, the heart began to beat violently, and continued to do so for a few moments, and the palpitation then passed off. Since that time an symptoms have been increased. Had but little trouble with the disease in heart since, and the few slight attacks have been immediately relieved by it. Pulse, intermittent. Throbbing, as in an abscess, in the stomach; after a pressing pain there; then throbbing in arms. Strong pulsation through the whole body. External Chest Tenderness over the second rib of the left side when touched; tenderness upon pressure, later above the left clavicle. An intermitting sore pain along the course of the third rib of the left side, which continues sometimes a minute (but more frequently less than a minute), and is sometimes accompanied with more stinging pain in the region of the sixth or seventh rib of the same side, which is sometimes felt in the elbow, at long, irregular intervals. In the morning, sore pain in the right chest under the nipple when bending forward, in sitting (second day). Neck Pain on the side of it. Burning on the right side of the neck. Back Between the shoulder-blades, stinging. Stitches pain from upper left chest through to the back between the shoulder-blades, worse at night in bed; awakens him, increases when lying on left side, not by taking a long breath. Pain on the right side of the spine, near the spine, exactly there where the shoulder-blade ends. From 2 o’clock P.M. until evening, tearing pain in the back, at the lower angles of the scapulae, when sitting still, not when moving. Under the shoulder-blade. Pain on both sides of the spine on a line with the inferior angles of the scapulae, with every deep inhalation, all evening. On each side of the spine above the middle of it, a spot where the breath appears to come from, drawing a long breath. Feels as if her back would break; can scarcely get up. In back the weakness is most felt. Sensation as of cold water running down the back. In back, warmth spreading. In small of back, burning. In the morning, fifteen minutes after the fifth dose, a kind of cramp in the back just below the hips on both sides; very warm ebullitions; the sensation goes from the hips up the left side; it frightens him and he quickly takes a cup of coffee. Upper Limbs Great weakness in the region of the shoulder-blades, with inclination to bend forward, which relieves. Severe pain in the left shoulder. Pains in the left shoulder, which disappear from motion and exercise. Itching on the right shoulder. Drawing-tearing pains particularly in left shoulder. Feeling of weakness, a kind of numbness in the left arm, and particularly in the left shoulder. In arm, lack of power and dull pain. In arms, fullness; of blood vessels. Between three and four o’clock I felt a pain in my right arm as though a cord were drawn tightly around it, just below the shoulder, occasioning great fullness of the blood vessels and deprivation of strength. The pain was dull and heavy, not general throughout the arm, but rather in lines, extending in different directions from the ligature which apparently bound it; the weakness was such that I could scarcely hold the needle which I was using when the pains commenced; it began suddenly, continued for a few moments, and then abated, although it did not leave me entirely, and returned at irregular intervals during the remainder of the day and evening. Renewed doses increased the pain in the arm, which is this evening quite severe. Pin in the right arm as if a string were tied tightly around it. Half an hour after the second dose pains in the arms and pains over the left eye. Rheumatic pains in the bones of the arms, particularly in the right; the right side of the chest and under the left shoulder blade. In the morning, for an hour, unpleasant sensation in both arms, afterwards heat, more in the left hand. In arm, pain from elbow; in elbows. In the morning, when awaking, rheumatic pain in the bend of the left elbow-joint, when extending the arm; disappears after rising and going about. Bruised pain in the joints of the arm, hand, and feet. Pain as if sprained in the right wrist, at 12 M., from thence the pain goes into the first phalangeal joint of the right forefinger, where it is a tearing, which disappears soon, but returns quickly (third day); felt weak in the hands, during catamenia; she drops things easily that she holds in her hands. Rheumatic pain in the left hand and all the finger joints. Hands pain; cold; heat; trembled very much. Finger-joints pain, first phalangeal joints. Lower Limbs Great lassitude in the lower limbs when going upstairs; the whole of the third day. In lower extremities, lassitude. Down into thighs, pain from rectum. When standing, bruised pain in the lower extremities, as after a severe cold. Uneasiness in the knees, with pain right above them. Pain in both knees, particularly in the right, when walking. Sensation as if a cord were ties around the leg, under the right knee; the pain extends downward and upward, but is confined to the lower leg. To-night a similar feeling, though much slighter, in the lower limb on the same side, as though a cord were bound round it below the knee, the pain extending both below and above, on the under part of the limb only. In the evening a kind of numb sensation in the right leg, from the knee down into the foot, when lying on it (one hour), (first day). Legs, from knees down, cold. Pain in the left ankle-joint when walking and particularly when going upstairs; in the morning (third day). Pain in feet. Coldness of the feet, with numbness, in bed, even if the feet are covered up warm. In the feet, coldness from the knee. Feet are very sensitive to pressure, and when walking, particularly the soles. Pain in the sole of the foot between the ball of the little toe and the heel; also, drawing, tearing pain, particularly in the left shoulder. In the evening, when walking in the street, stitch pain in the under part of the right big toe; felt each time when pressing on it, all evening (second day). Stitches in the right big toe. All the Limbs Rheumatic pain in the whole body. Pain in the right side of the neck and forehead, then in the left foot and ball of the foot, then on the vertex. Bruised pain in the whole body. Slept well; in the morning, an hour after rising, tiredness, and for three hours a twitching in the limbs, particularly in the arm, and with it much excited. Coldness of the legs from the knees down to the feet, and coldness of the hands, in the afternoon and evening. Coldness of the hands, and rheumatic pain in the left shoulder; worse after motion. Cold hands and feet, as if all the blood had left them, with pain in stomach. Generalities Has to throw himself about in bed; restlessness, agitation, with fever. Nervous tremor and excitability of feeling, as when one is agitated by unpleasant news. Trembling of muscles. Twitching in limbs three hours in the forenoon; of lower lip. Jerking contraction of abdominal muscles; follow the warm creeps. Slight convulsive movements. Sensation as if a string were tied around the arm and lower limbs. Weak feeling in pit of stomach; sinking sensation; uneasiness; faintness and weakness, as if dying. Lassitude in the whole body, particularly in the left lower extremity (Trbg). Feels very tired (soon after taking it). Keeps the bed; so faint that she expected every moment to fall from her seat; (reached her chamber with difficulty; staggers to a chair); deathlike fainting. Weak with the nausea; could not sit up; loss of strength, with sickness; faintness and nausea (in the morning). Very weak, as if she had been ill for some time; great weakness, as after long illness; debility in typhus. I was immediately sensible of a metallic taste, and slight nausea, but soon fell asleep, being greatly fatigued, and slept heavily until near five; I arose without more effort than usual, and about six, while standing at a table making some arrangements for breakfast, was suddenly seized with the most overpowering nausea, as though my whole stomach were rising to my throat; I tottered to a chair, when a cold sweat broke over me; the effort to vomit ceased, though the nausea continued, attended by a flushing to my head, extreme faintness, and a disposition to shiver; I reached my chamber with much difficulty, and on lying down felt some relief, chiefly in the faintness, and after taking some Ipecacuanha was able to rise and attend to my family, though feeling very weak; strong palpitation of the heart, intermitting pulse; the pain shoots from the heart to the region of the lower right lung. Great weakness after attack, particularly in spine. He feels exactly as he felt when he had the small-pox (second day). Sleep When going to bed heat in ears and face disappears. Cannot get to sleep on account of ebullition of blood (first day). Is more anxious at night than at other times. Sleeplessness, with fever. Very light sleep in the night; hears nearly everything that happens (Trbg). Very restless sleep, tossing to and fro; when he does sleep it is so light that he hears it if his birds move in their cage (Trbg). Wakes at night, with night, with nausea and violent headache; lay awake about a quarter of an hour, when he went to sleep again; next morning, when he was called, he was unable to attend to his business on account of nausea, with violent headache; now sharp stinging, from temple to temple; now heavy pressure on the vertex, as of a heavy weight lying on it; continued all day long. In the night, half waking, half dreaming, she hears what happens around her, but dreams on, of dead bodies, that her father had died; with it great fear; would like to turn on her side, but cannot do it, feels so heavy in her bones; in her dream it seems to her as if the dream would soon pass over if she could turn on her side, but she is not able to do it (at 4 A.M., fourth day). When awaking in the night, thick phlegm in the throat, and roof of the mouth thickly coated, which she never had before (third day). Inclination to chilliness after attack of retching. Chilliness with the sickness at the stomach. She complained of being very cold, but did not shiver; it passed off, perhaps in an hour, but was succeeded by pain in the head, which lasted all the afternoon. In the last attack the coldness was general; she could not distinguish a particular coldness of the feet; but all perceptive faculties were gone during the deathlike fainting spell. Chilliness, and feeling as if cold water were poured down the back. Coldness in hands; of legs and hands. Cold feet, even when covered up. Sensation of warmth, soon after taking it. Great heat throughout the body. Sensation all forenoon, as of inward heat. Heat and tingling following creeping and coldness. Warm ebullitions in both sides. He cannot get to sleep for a long time, on account of extraordinary ebullition of blood in the whole body (Second day). In the morning, after the fourth dose, very soon tired; warm ebullitions like spasms spread over the whole trunk, particularly in the back; no pains, but a trembling of the muscles; towards two o’clock the pains in the chest return; feels better when walking. Flushes of heat to the head after attack. Warm creeps run over the abdomen. Cold sweat; breaks out over the whole body; covers her, with cramps in stomach. Chilliness; at another time, great heat throughout the body. Coldness, with pain in the stomach, gave place to heat, and the blood rushed to her head with such force that she scarcely could hold it up. Chill; metallic taste, slight nausea, palpitation; pain striking from the heart across to the lower extremity of the right lung; a sensation as of cold water poured down the back, and numbing coldness of the feet, though covered warmly in bed (returned repeatedly after taking the 12th); in the last attack the chill was general; I was not sensible of any particular coldness of the extremities, though, indeed, nearly all sensation was lost for a time in the deathlike faintness. 10.0 Alumn : Alumen   REDUNDANT SIGMOID COLON Book INDIAN JOURNAL OF HOMOEOPATHIC MEDICINE By Praful Barvalia. Volume 1995 Vol 30 No 3 Author Mustafa S Hakimji. Subject Cases Remedy Ambr / Alumn / Syph. A CASE STUDY Dr. M.S. Hakimji L.C.E.H. CASE: Master Murtuza aged 3 years, a Muslim child residing in Singapore was brought to me on 27.2.86, C / o. Chronic constipation since infancy. The child was given a lot of allopathic purgatives since infancy for his ailment. At present he could pass his stools only after an enema. The stools were hard, dry, offensive, occasionally in form of round balls. Mucus was absent. The Child had to strain a lot during defaecation. The child preferred to pass stools, standing. Child also occasionally complained of epigastric pain and vomiting. He did not like anyone around while passing stools. He was slim & tall for his age. A poor eater he had a strong dislike for solid foods and fruits. He liked cold drinks, aerated waters, lemons, pungent things, soups and salt. His thirst was as less as his appetite. A hot child, he was uncomfortable in the sun and preferred a cold bath. The child was born Caesarean and breast fed till 6 mths. Supplementary feeds were started from 6 months onwards. The child disliked the bottle and had to be spoon-fed. The childs milestones were normal. A playful, quiet and intelligent child, he was extremely apprehensive of being examined. That I presumed was due to the fact that the child had been seen by various doctors before being brought to me. According to the mother, the child used to get frightened easily. He was a light sleeper awakening every two hours before going back to sleep. There was no past history of any major illness. Family history revealed that the mother had suffered from 2 miscarriages prior to the birth of the patient and had hypertension and urinary tract infection during her pregnancy. She had also suffered from meningitis and jaundice during her childhood. The paternal grandmother suffered from gout, while the maternal grandmother had hypertension, rheumatic pains and ischaemic heart disease. The maternal grandfather suffered from rheumatoid arthritis. ON EXAMINATION: T.R.P. (N) Tongue: Clean & Moist. Teeth: Upper incisors cupped. P / A Soft – full of faecal lumps. Guarding + Peristalsis + INVESTIGATIONS: Barium Enema of 11 / 12 / 85 was suggestive of Redundant sigmoid colon. DIAGNOSIS: Chronic constipation due to Redundant sigmoid colon. The child was advised surgery by the Orthodox School. Could we as homoeopathic physicians do some thing? Well, the treatment commenced with a single dose of Ambra grisea 30 and sac lac pills there times a day. The child was all right for two days. Ambra Grisea 30 was repeated and the problem resolved again. Since the child was going back to Singapore, I gave him both Ambra Grisea 30 & Sac Lac pills, asking the parents to give Ambra Grisea only if improvement stopped. They were asked to report after one month. A month later, I received a letter stating that the problems continued as earlier. He passed the stool only while Ambra Grisea was given. This time I also received the history that his food was being prepared in aluminum vessels. I sent word asking the parents to stop cooking in aluminium vessels and a few doses of Alumina 30. Alumina 30 was a wrong hasty prescription. The very next day I received an urgent call saying that the child was very bad, vomiting and having severe tenesmus. Alumina 30 was stopped forthwith & an enema was given to relieve the child. It was then I decided to give what was upper most in my mind, and which if you go through the case carefully is the only remedy indicated in this case. Syphilinum 1M, 1 dose was given on 23.5.86 and a week later 1 more dose repeated. The childs problem resolved as if a magic wand had been passed over him. He passed stools freely, though only after 4 days. Besides that his shyness also diminished. Till today he remains free of his problem which plagued him from infancy. DEPARTMENT OF PRACTICAL CASE MANAGEMENT [Case Reports]   Book THE HOMOEOPATHIC RECORDER By Rabe R F. Volume 1939 Dec Vol LIV No 12 Author Eugene Underhill. Subject Cases Remedy Alumn. EUGENE UNDERHILL, JR., M.D., Editor. CASE REPORTS. CASE I. Male, age 57, married, 3 children; occupation: garage business; weight 144 lbs. Chief complaints: loss of appetite, chronic indigestion and constipation; duration twelve years. Has tried all sorts of treatment and diets without relief. Becoming increasingly weak and emaciated. His wife was informed by at least one physician that her husbands trouble was undoubtedly cancer. Patient was thin and anemic in appearance, bordering on cachexia. Mentally very irritable and brooding much over his condition. Complained of feeling dazed and confused in his mind. Unable to decide matters pertaining to his business. Compelled to lie down and rest several times a day. Unable to be actively engaged in his work and reaching a point where he could no longer give his business adequate supervision. Rumbling, gurgling noises in the abdomen were frequent and annoying. Chronic constipation for many years. Much ineffectual urging, the stool being variable, sometimes hard, sometimes soft, often nodular and lumpy. Dryness of mouth with a metallic taste has been a persistent symptom, also dryness of the skin with annoying itching. A feeling of weakness and uncertainty in the back and legs has been increasingly complained of with easy fatigue on walking. Phantom tumor of the abdomen has appeared from time to time in various locations probably due to incarcerated flatus. Pending selection of the remedy suitable dietary correction was instituted but without relief. Study of the case resulted in the selection of Alumina as the indicated remedy. This selection raised a question as to whether the patient needed potentized Alumina or whether he was being poisoned by the metal. Upon inquiry it was discovered that aluminum utensils were used at home in the preparation of food but in order for this to be etiologic significant the patient would have to be allergic to aluminum. As a precautionary measure the discontinuance of the use of such utensils was advised. Fortunately in marking a re- examination a discovery was made that had been overlooked the first time. The patient worse an upper denture the base of which had a bright metallic appearance. Upon inquiry the patient stated that the denture was mounted in aluminum. How long had he worn this plate? Twelve or thirteen years. How long had he been suffering? About twelve years. Instead of prescribing Alumina the immediate discarding of the aluminum denture was advised. This proved to be the turning point in the case and a rapid and uneventful recovery has resulted. Improvement began in less than two weeks after the aluminum poisoning was stopped. Now after several months everyone says that the patient seems better than he has for years. He no longer suffers from indigestion; the bowels are regular and he is looking after his business every day. We were doubtless more fortunate than wise in discovering the true etiology of this case. The unusual appearance of the denture had escaped us at a previous examination. This serves to emphasize the importance of a careful physical examination as a routine procedure in every case. CASE II. The only possible excuse for this case was that it occurred many years ago. A man who had enjoyed shad for dinner complained of a fish bone sticking in the roof of the mouth. Examination with a good light disclosed nothing and the patient was informed that the sensation might persist for a while after the fish bone was gone. In a short time he returned with the same complaint in the same spot on the palate. Again no fish bone was discovered even by means of a digital examination. Just why he returned the third time will always remain a mystery. He should have been hunting for another doctor. “Its still there, Doc,” he said, “right there”, pointing to a spot on the hard palate. “I can feel it every time I rub my tongue over it”. Luck was with us for when he mentioned the word tongue it actually did occur to us to examine the tongue and sure enough there was the fish bone as plain as day. QUESTION AND ANSWER DEPARTMENT. [ SEnd questions to Dr. Eugene Underhill, Jr. 2010 Chestnut Street, Philadelphia, Penna]. Question: WHAT IS THE MOST EFFECTIVE REMEDY IN HOMOEOPATHY FOR WEANING AN OPIUM ADDICT? DR. A. S. RAJAH, SINGAPORE. Answer: Probably the most effective remedy for weaning an opium addict is Nux vom. high, and next, Opium high. Sometimes Ipecac comes in and occasionally Lycopodium, with the emaciation from above downward that so often occurs in these patients. The most effective remedy is, of course, the most similar, and each individual personality must be taken into consideration, and the reaction of that patient under the narcotic itself. Dr. H.A. ROBERTS, DERBY, CONN. Question: IS THERE ANY SPECIAL HOMOEOPATHIC TREATMENT FOR OPIUM SMOKERS? DR. A.S. RAJAH, SINGAPORE. Answer 1: The treatment for opium smokers would naturally be first antidotal and then constitutional-in other words, first the treatment for an acute condition to be followed by the chronic remedy-and the remedy may be the same of different. In the constitutional treatment the above statements would hold as well as for weaning the patient. In the excitable stages have particularly in mind Belladonna and Hyoscyamus, with their clear indications: Lachesis and some of the snake venoms in the more subtle cases, especially Naja with its nervous reactions. In the acute states of wakeful excitement Coffea. In the exceedingly childish states of irritability Chamomilla. In the downright sulky ugliness Nux vom. Do not think for one minute that these remedies are all that might be indicated, for there are many that may be called for. Opium has a most profound effect on the very character of an addict, affecting as it does the mental, nervous and physical system of the patient. DR. H.A. ROBERTS, DERBY, CONN. Answer 2: Pulsatilla and Muriatic Acid for the diarrhoea, Chamomilla for nervous states. After the first remedies Calcarea sulph. and Kali phos. help build up the nervous system. Any of the five may be needed for the condition, depending upon individual symptoms. In my own experience Calcarea sulph. has been my best remedy. DR. A.H.GRIMMER, CHICAGO, ILL. Subject Cases Remedy Alumn The case reported by Dr. Elliott in November issue is remarkable in many particulars. One of the most remarkable features was the persistence of the Doctor in letting good enough alone. He evidently “knows a thing or two” of the human system and of Homoeopathy. I wish to record a case now under treatment. Mrs. B., aet. fifty, apparently well for her, of nervous temperament, has had a lifetime of constipation. Her brother-in-law is a prominent allopathic surgeon in Chicago, and has racked his brain and bothered his colleagues not a little to relieve this lady. The whole list of purgatives have passed through, some singly, with a slow plod, and others in combinations innumerible, have gone with a “hop-step-and-jump,” sweeping everything before them, and not stopping for two or three days afterward, but the old trouble returned; so, for years she did nothing. The “enemas” or “clysters,” in reality rectal injections, were used on special occasions. It was her habit to go two or three weeks without movement, and of late years she very frequently goes six weeks with no inclination to movement. This interval has been common for several years. Then she empties out in two or three days and dispenses with that function for another such interval. I have given her an occasional prescription for some new trouble, and perhaps one call would suffice for the time. She has been abroad for months at a time. At my request she called on Dr. Skinner, but twice found him out. She called on a physician in Germany who gave her one powder, with a request to call again “in four weeks,” but she received no effect and did not see him again. On inquiry she stated that during the interval of bowel suspense the urine was much increased. The stools are not peculiar. About two weeks since I gave her a dose of Alumen 1m (Skinner), but have had no report since. In Vol. IV, p. 238, of The Clinique, Dr. Bailey has detailed a remarkable case in which Morphia hypodermically seemed to be the only agent used which produced a movement of the bowels. The intervals were: “February 13th to April 28th, unchanged, no movement of the bowels whatever; May 1st to 28th; May 30th to June 17th; June 30th to July 13th.” He there states that the constipation appeared soon after the disappearance of an abscess in the lower portion of left lung, (probably.) Phthisis was hereditary. In Vol. V, p. 449, of same journal, he gave a subsequent report of the case with post-mortem record. “After an illness of more than five years my patient died, October 31st, 1884, of phthisis pulmonalis. The bowels were inactive, the intervals varying from one to nine weeks.” Atrophic changes were marked in various organs, “the peritoneum was as thin as tissue paper and glued by adhesions to the intestines. The stomach was divided by constricting peritoneal bands, so as to separate it into two quite distinct compartments of nearly equal size, and as an organ was enlarged and distended. The omentum was firmly adherent, producing a serious knotting together of the entire small intestines. The transverse colon was greatly distended and enlarged, and pouched by constrictions the same as already noted. At the splenic flexure there was a great band, which by encircling the colon had caused an abrupt narrowing of its calibre, as great as the change in diameter of a wrist to that of an index finger, which was literally true in this case. This narrowing of the descending colon was continued until it had reached the signoid flexure; here a congenital malformation was noted, the colon traversing across the lower abdomen, a curve being found at a point just over the ilio-coecal valve, from thence passing downward and backward terminated in an enormous pouch which completely filled the inferior outlet of the pelvis. All shape of the rectum had been lost. The bladder was shrunken to the size of a walnut. The constipation was a result of pathological changes, and in part congenital. The disease was tubercular.” My patient does not show a tubercular tendency, but may later. I think the above will give us hints in the study of such cases. Subject General Topics / Cases Remedy Alumn In 1899 Mr. E. reported that he had had eczema between the shoulders, for years, temporarily removed, at times, by Sulphur paste. Once had several boils with roughness of the skin in that part. About four years previously he had pain and pressure in the rectum. He supposed that a hemorrhoid had protruded but found it was easily returned within the rectum. About three years after its first appearance, this growth became very large and painful. After an allopath had prescribed lead lotion, and a mongrel homoeopath diagnosed “piles,” and advised their removal, neither advice being followed, Hazeline was employed. The tumor became much worse, with intense pain after stool. Six months previous to the visit, an allopath made an examination, the first that the patient had been given. He discovered, and removed, a polypus with its stem beginning half way up the rectum. He said there was not the slightest chance of a return of the trouble. Four or five months later, the patient became more constipated; one day he thought he had strained the rectum, during evacuation, after which he used glycerine enemata with benefit. He said he had no faith in Homoeopathy, but one of my patients had advised his consultation, as surgery and allopathy had failed. I replied that in Homoeopathy works precede faith, and that I was glad that he did not believe, because after he was cured, no one could say it was the effects of faith or of imagination. 1900. Nov. 11. Rectum-pain in region of internal sphincter, as if strangulated, since August, recently, much worse; Paroxysms begin 1/2 to 1 hour after stools; sometimes continue all day, and even into the night. Burning pain about anus. Shooting pain from anus extending down one-third the inner side or left thigh; not affected by glycerine enemata. Before polypus was removed, pain aggr. when standing. Diagnosis of the Remedy: The keynote was the rectal pain extending down the thighs. This belongs only to Alumen (Kent’s Repertory and symptom 192 in Allen’s Encyclopaedia). This remedy has also burning pain in anus, and hard stools. Constriction during evacuation belongs to the allied remedy, Alumina; but the provings of Alumen are yet incomplete. Alumen 1m. F. C. One dose. Dec. 16. Rectal pains improved almost immediately, and in about ten days had almost ceased; entirely absent for past two weeks. Burning around anus absent for past two weeks. Faecal evacuations improved in about ten days; now nearly normal. 1900. Jan. 30. No more rectal pain. Bowels normal. He now firmly knows that Hahnemannian Homoeopathy is true. There was no return of rectal trouble to time when last seen, Dec., 1906. He has remained in my care, and sent his family and friends, also, to me. His subsequent history may be of interest. 1899. Dec. 27. Cough, following cold effects, forces him to kneel on the bed, with head downward. Eup. perf. cm. F. Cured. 1902. April 10. Remained in perfect health until re-vaccination with calf-lymph, last November. Arm very sore for five or six weeks; pustules, very offensive odor. Influenza about six weeks later, which was easily thrown off. Hemorrhoids left side rectum, followed influenza; Pain as if cut open with a knife. Relieved by use of Arsen. 3. Influenza again, about four weeks ago, followed by re-appearance of Hemorrhoid. Pain prevents sleep past three nights; Aching extends to 1. natis. Diagnosis of the remedy: Repertory gives thirty-nine remedies under cutting in rectum. Of these, Silica and Thuja are the chief antidotes to the vaccine virus, and Silica occupies first rank as to the cutting pain. Silica 6m. F. C. One dose. May 7. Cutting pain decidedly improved the same day of last report; in two days, it and aching in 1. natis entirely ceased. 1906. Dec. 3. No return of rectal trouble. Asthma, past few years, beginning near end of October. This year, more intense and longer continued. 5 A. M. or earlier (to-day, 3:45 a. m.) wheezing and coughing for an hour. Cough forces him to sit up and lean forward to r. side. expectoration scant. Symptoms usually continue for an hour; entirely absent by 10 A. M. Chest wooden sensation, as if no elasticity. Soreness 1. side, these two days. Heart pulsation rapid. No desire for smoking, past two months. Irritable when rising at 8 a. m.; disposed to quarrel; amel. after an hour. Diagnosis of the Remedy: Repertory gives eight remedies for irritability when rising in morning, two remedies for asthma. 4-5 a. m. Natrum sulph. also belongs to both. Other symptoms were not especially characteristic or were common. Natrum. sulph. cm. F. One dose. Dec. 20. Has had influenza for two weeks, but has now thrown it off and is much better. Asthma was improved, the day after the remedy was taken. Wheezy from 7 to 8 a. m., yet Respiration difficult until 9 a. m. Chest constriction across upper part not entirely disappeared. Sleep good; does not waken until 6 A. M. Generally much improved and enjoys his pipe. Irritability unchanged. No complaints since. (1) This case illlustrates the selection of the remedy from the key-notes alone, but this should be done, only when other symptoms of the case are common, or when the remedy has not been completely proved. (2) Surgical operation accomplished nothing. The only relief was a mechanical one: the pain not then aggr. by standing. (3) The peculiar cough symptom of Eup. per. I have verified, clinically, in three patients. (4) The effects of vaccination are suggestive. Boenninghausen considered variola and vaccine both belonging to the Sycotic miasm; for which he places Thuja in the first rank, and Silica its second, but Silica occupies first rank in the cutting pain. Hence its selection. (5) The effects of “mark of the beast” appears to have continued longer, for Natrum sulph., which relieved the asthma, is also sycotic. Perhaps Silica expended its energy chiefly on the evident Psoric taint.
